Patient with -18 diopters completely ruins left eye with PRK


Just crazy!


I am not a doctor but I can comment. He got PRK for nothing! This was clearly malpractice to attempt the impossible. PRK is not even approved for more than -6 to -8 diopters because the risks get unacceptable. His was three times too high. Why didnt he just get IOLs or wait for something better?

At this point, I would not touch the right eye anytime soon if ever. He is legally blind in the left and has only one good eye. What needs to be done is try to resolve his haze. Once thats out of the way, I would try to reduce his cylindar as much as safely possible and as much as theres enough cornea left with more PRK. There is little to lose as hes already legally blind in the left and if it cant be repaired, he would be looking at a possible cornea transplant. This would bring him back to square one. Then glasses can be worn again without the great anisometropia issue.

IOLs would also take care of the anisometropia but its risky because one eye is ruined. If anything happens to the good eye, his quality of life would be impaired, he would not be able to drive or do much. Its of course best not to touch the good eye and try to repair the bad eye and if that fails, a cornea transplant. It was clearly a case of malpractice and the patient was uninformed and trusted the doctor thinking he could do things right and reduce his dependancy on very thick glasses.

His UCVA was clearly worse than 20/400 or even 20/800. More like 20/4000! His BSCVA of 20/25 is unusual given the spectacle minification and how bad/myopic his eyes were. Naturally, he wanted to do something to reduce his myopia, improve his vision. PRK was clearly inapproperate.
